摘要 腔鏡技術(shù)在胃腸、肝膽、泌尿、胸外科等領(lǐng)域的安全性及有效性已經(jīng)得到驗(yàn)證,相比傳統(tǒng)手術(shù)方式具備顯著優(yōu)勢(shì),在臨床中已廣泛應(yīng)用 [ 1 ] 。乳腺作為體表實(shí)體器官,在腔鏡技術(shù)的應(yīng)用方面起步較晚,與其他具有自然腔隙的腔鏡手術(shù)不同,乳腺及腋窩本身沒有腔隙,需要人為建立手術(shù)操作空間。1992年Kompatscher教授 [ 2 ] 通過腔鏡技術(shù)取出乳房整形術(shù)后攣縮的假體,是腔鏡技術(shù)在乳腺外科領(lǐng)域的首次應(yīng)用。1995年Friedlander等 [ 3 ] 通過腔鏡技術(shù)切除乳腺腺體和腋窩淋巴結(jié),并利用腹直肌進(jìn)行乳房重建;1998年Kitamura等 [ 4 ] 提出應(yīng)用腔鏡技術(shù)切除乳腺良性腫瘤,采用腋中線三孔入路,利用擴(kuò)張氣囊鈍性分離聯(lián)合CO 2灌注的方法建立腔隙完成手術(shù);2001年Tamaki等 [ 5 ] 首次提出通過乳暈入路切除乳房?jī)?nèi)象限腫塊,解決了腋窩入路操作路徑遠(yuǎn)、手術(shù)難度大的問題。隨著腔鏡乳房手術(shù)(endoscopy-assisted breast surgery,EABS)的不斷發(fā)展,目前臨床已開展乳腺癌保乳術(shù)、保留乳頭乳暈的乳房切除術(shù)、乳房重建術(shù)、腋窩淋巴結(jié)活檢術(shù)等多種手術(shù)方式,為乳腺外科治療提供了更多選擇。 腔鏡技術(shù)開拓了乳腺疾病治療的新領(lǐng)域,通過微小且隱蔽的切口實(shí)現(xiàn)準(zhǔn)確、精細(xì)的手術(shù)操作,在保障腫瘤安全性的同時(shí)更加美觀。在乳腺癌的治療中,腔鏡技術(shù)應(yīng)用于保乳術(shù)、皮下腺體切除術(shù)、乳房重建術(shù)、前哨淋巴結(jié)活檢術(shù)及腋窩淋巴結(jié)清掃術(shù)等,可行性及安全性與傳統(tǒng)手術(shù)相當(dāng),具有術(shù)后美容效果好、切除準(zhǔn)確性高、術(shù)后并發(fā)癥少等優(yōu)勢(shì)。但仍存在需要更復(fù)雜的手術(shù)器械、手術(shù)時(shí)間長(zhǎng)和手術(shù)花費(fèi)多等劣勢(shì)。腔鏡技術(shù)在乳腺良性腫瘤、男性乳腺發(fā)育癥及腋窩副乳腺切除中也具備一定優(yōu)勢(shì)。目前腔鏡技術(shù)未能在乳腺外科領(lǐng)域全面普及,可能與學(xué)習(xí)曲線長(zhǎng),缺乏腔鏡手術(shù)器械等原因有關(guān)。但微創(chuàng)乳腺手術(shù)技術(shù)正在不斷革新并逐漸標(biāo)準(zhǔn)化,相信在不久的將來乳腺微創(chuàng)理念和腔鏡技術(shù)在乳腺疾病外科治療領(lǐng)域會(huì)有更廣闊的發(fā)展前景。 (在框內(nèi)向上滑動(dòng)手指即可瀏覽全部參考文獻(xiàn)) [1] Hashmonai M , Cameron AE , Licht PB ,et al.
